Request for Transfer Credentials

POLICIES AND GUIDELINES:

Transfer Credentials are to be processed when a student intends to transfer to another school. For a particular student who leaves the school, this document is issued “once” only.

The original copy of your Transcript of Records will be mailed to the school where you transferred upon receipt of request letter from said school.

PROCEDURE: 

  1. Secure a clearance form at the Registrar’s Office.
  2. Process your clearance (to be signed by the proper school authorities) and pay the required fees at the Accounting Office.
  3. Present your receipt of payment to the staff-in-charge of Transfer Credentials and be advised when to come back for your request. (to be released not later than two weeks from the date of filing of the application – based from MORPHE, 2008)
