POLICIES AND GUIDELINES:
Request for OTR should be done personally. Parents may request for the OTR of their son/daughter if such is still a minor, otherwise an authorization letter from the owner of the record should be presented.
Note: Regular processing of OTR is 15 working days from the date of request. However, the OUR also accepts “express processing” (one-day processing)
PROCEDURE:
For the First Request:
- Get a clearance form at the Registrar’s Office, accomplish it and have it be signed by the proper school authorities.
- Before paying at the Accounting Office, let the Records Clerk at the Registrar’s Office indicate on your clearance the amount (TOR fee) to be paid at the Accounting Office. Said amount depends on the number of pages of your transcript of records
- Note: Please inform the Records Clerk if the TOR that you need is for Board Examination purposes or for general purposes.
- Submit your clearance and receipt of payment to the staff-in-charge of OTR and be advised regarding the release date of your TOR (not later than two weeks after filing of application – MORPHE, 2008)
- Claim your request personally. If not, an authorization letter and a valid ID should be presented by your representative.
