POLICIES AND GUIDELINES:
A Diploma is issued once only. However, if the diploma has been lost or damaged, a second copy may be issued but an “affidavit of loss” should be submitted.
Note: Regular processing of Diploma is 15 working days from the date of request. However, the OUR also accepts “express processing” (one-day processing)
PROCEDURE:
- Get a copy of a clearance form from the Office of the University Registrar, accomplish it and have it be signed by concerned school authorities.
- Present the duly-signed clearance to the staff-in-charge of diploma at the OUR and be advised when to come back to claim your diploma.
- Claim your Diploma personally, otherwise an authorization letter and a valid ID should be presented when claiming is done through a representative.
