Request for Certifications

POLICIES AND GUIDELINES:

The Office of the University Registrar is authorized to issue the following certifications:

  • Grades in previous semester
  • True Copy of Grades
  • Enrolment
  • Units earned in College or in the Graduate School
  • Weighted Average of grades
  • English as a medium of instruction

PROCEDURE:

  1. Fill up an application form for the certification being requested.
  2. Present your application form at he Accounting office and pay the required certification fee
  3. Present your receipt of payment and application form the staff-in-charge of issuing certifications at the Registrar’s office.
  4. You may wait for the release of the certification (subject to the availability of the Registrar to sign the document)
