Grade 7 Applicants:
- Clear photocopy of NSO Birth Certificate
- Clear photocopy of report card (Form 138) with a general average of 80% and above
- 2 pcs. latest colored I.D. picture (2″ x 2″); white background with name tag
- Testing fee of P100.00 (non-refundable)
Transferees:
(Incoming Grade 8 students only)
- Clear photocopy of NSO Birth Certificate
- Clear photocopy of report card (Form 138) from previous school with a general average of 85% and above
- 2 pcs. latest colored I.D. picture (2″ x 2″); white background with name tag
